Acentra Health is assisting the West Virginia Department of Human Services’ Bureau for Behavioral Health (BBH) with conducting annual consumer satisfaction surveys on behavioral health services that West Virginians receive any time between October 1, 2024, and August 15, 2025. BBH reviews this feedback for systemic improvement and sends a summary to the Substance Abuse and Mental Health Services Administration (SAMHSA) as part of its SAMHSA block grant report.
There are three surveys: one for adults who received treatment; one for youth aged 12-17 to complete themselves; and a third for parents or guardians of children and youth who received services. Individuals may take the surveys online or by paper as follows:

- Youth survey online or print 2025 YSS (Youth) survey
- Family of youth survey online or print 2025 YSS-F (Family) survey.
